Privacy Policy & Your Information

 

In accordance with the General Date Protection Laws (GDPR) 2018, I am required to clarify to all potential clients, how I will store their data. 

I am registered with the Information Commissioner’s Office, and I treat all my data according to GDPR requirements.

 

Enquiries only

 If you contact me about an enquiry,  I will not keep a record of your personal details, and I will not contact you in the future.  I will await you to contact me again, should you wish to book an appointment with me.

 

Current Therapy Clients

 Should you decide to make an appointment with me or continue into therapy,  I will keep a record of information that you choose to give to me.   

 

What information will I keep? 

Contact details: name, address, telephone number, email,

Personal data: date of birth,

Third party information:  GP details, Psychiatrist, Mental health worker, Carers,

Sensitive information: therapy notes, questionnaires

  

How is your information stored? 

Whilst you are in active therapy, Contact details will be stored on a password protected mobile phone. This device is only used for clinical practice purposes and will always be kept in a secure place.

Email details are kept on a password protected personal computer. No one else has access to this device.

Written Therapy notes are stored in a secure locked filing cabinet.  Only your therapist has access to this confidential information. 

Electronic therapy notes are stored on an encrypted hard drive, which is kept locked in a filing cabinet. Only your therapist has access to electronic confidential data.

  

How will I contact you between sessions? 

After a next appointment has been agreed, I will only contact you if I need to rearrange our appointments.

I will respond to communications between sessions, if you contact me first.

 

Should I need to contact you, I will always try to use your preferred contact method;  phone, text or email. 

 

How will I share your personal information? 

All of the information you share with me will remain confidential, unless specified below:

       

-          If you request that I share your information with a third party,

 

-          If Information is requested for the courts. 

 

-          If I am seriously concerned about your mental health, or you tell me you have plans to harm yourself,

 

-          If I have serious concerns about your risk to others,

 

-          If your therapy is covered by health insurance, your insurer may request information such as outcome measures or an update on your progress.  All contact with insurers will be through password encrypted email or telephone.

In every circumstance I will endeavour to discuss any information sharing  with you first.

 All information sharing will be done through the most secure method of data sharing where possible.  

  

Past clients

 I am professionally obliged to keep your details in storage as per medical records, for a 7 years after the end of therapy for adults, and for 7 years after the 18th birthday of a child.     Written data will go into secure storage cabinet, and electronic data will be kept on an encrypted secure hard drive which is locked in a storage cabinet.

I will delete your contact details from my telephone and computer.

I will not contact you after your therapy has ended.  I will wait for you to contact me, should you require my services again in the future.
